After you make your first appointment, please take a minute to fill out the following online Patient Forms before you arrive for your appointment. Financial Information

Our primary goal is to provide you with excellent dental care. The cost of your treatment will vary, depending on your individual needs and treatment plan. We will always discuss the estimated cost of your treatment prior to beginning any care. Each of our available payment options will be discussed before we begin your care so that you can make the best choice for yourself and your family.

Our practice accepts personal checks, cash, credit cards, insurance plans and Care Credit.
